A brochure, price information and an application for enrollment are available at the
Children's Village office.

A completed application may be mailed or delivered to the Children's Village office. A non-refundable registration will be collected with the application.

Child's Preadmission Health Evaluation
Immunization record
Child's Preadmission Health History
Signed & Dated "Notice - Parent's Rights" Receipt
Student Enrollment Record & Consent for
Medical Treatment
Personal Rights - Day Care Facilities
